Dental Lab Work – do dentists or labs perform the final "fit checking" of your case?

I published the following information for dentists to review on YouTube, along with a short video.  Ultimately it is the dentist’s responsibilty to perform the final “fit checking” of your lab work prior to placing it permanently in your mouth. 

“Everyone knows that dentistry is not an “exact science.”  That being said, where do you draw the line regarding the fit of your lab work.  Do you simply trust that your lab is operating at 100% quality control?  Whose level of quality control?  I highly recommend that you at least purchase a stereo zoom microscope that can be mounted to a countertop in your office.  “Fit checking” all of your cases under a microcope after they come back from your lab, will give you the opportunity to send the case back for further adjustments.  You could be faced with making these adjustments directly in the mouth wasting your time, your money, as well as the patient’s time.  For years I’ve struggled with inconsistencies related to lab work during seating appointments.  I had to finally “grab the bull by the horns” and you can too.  Simply purchase a microscope and watch my video on fit checking.  I will be happy to give you the microscope information that you will need to make the right purchase.”  GC

The quality of my dental work  reached an un-imaginable high level of care once I stopped using dental loupes and switched to using a dental microscope.  I send all of my patients’ lab work to dental labs that also use microscopes.  Finally, I perform the final “fit checking” procedure and then place the lab work permanently in my patient’s mouth.  GC

Choosing the right dentist by asking the dental lab?

For years, I have accepted many dentists as patients for restorative treatment.   These dentists are seeking out the very best of quality dental care.  Here is a list of some common key points that they shared prior to selecting me as their dentist:

  1. The dentist must have many years experience in cosmetic and restorative dentistry

  2. The dentist must be meticulous in tooth preparation design

  3. The dentist must have persistent attention to detail

  4. The dentist must use a dental microscope

  5. The dentist’s lab must use a dental microscope

  6. The dentist must use a high quality (non-commercial) dental laboratory

Each dentist consulted a high quality dental laboratory and ask the lab owner for a referral as to who they thought presented them with the best dental work.  These dentists were also allowed to review my cases in the laboratory to help them make their final decision.  GC

Common Causes of Chemical Erosion

1. GERD, also known as gastroesophageal reflux disease, is a chronic symptom of mucosal damage. It is caused by stomach acid (hydrochloric acid) coming up from the stomach into the esophagus and eventually reaching the oral cavity.
2. Bulimia, an eating disorder, is characterized by binge eating and then purging (vomiting).
3. Carbonated beverag…e swishers show tooth wear on the facial (front) side of teeth. The fizz (carbonic acid) causes the demineralization of enamel to eventually form islands of the underlying tooth structure known as dentin. Colas are the most destructive, as well as 5 and 7 hour energy drinks (phosphoric acid) and Gatorade (high fructose sugar).
4. Citrus fruit suckers have front teeth that look like a screwdriver blade. The enamel erodes on both the front and back side of front teeth.
5. Citrus fruit mullers have more wear on back teeth than front teeth.

Please note: 25-50% of GERD patients may not have any symptoms other than dental. GERD is a precursor to Barrett’s esophagitis which can lead to esophageal cancer!! GC

CAD/CAM Dentistry – Computer-Aided Design and Computed-Aided Manufacturing in Dentistry

Old School vs. New School . . . CAD/CAM technologies produce different types of dental restorations, including crowns, inlays and onlays, veneers, fixed-bridges, implants, and orthodontic appliances.  These restorations are milled from solid blocks of ceramic and titanium materials.  Computer images are either captured by hand-held devices which scan directly in the mouth or desk-top which scan images outside the mouth.  The images are sent to the computer software where they are interpreted and set up for design.  At this point, the designers are either going to be your dentist, dental assistant, or dental laboratory technician depending on the system being used.  The virtual images are sent to the milling center where the final restorations are produced.  For further information, feel free to visit the following sites:  http://www.3shape.com/why-3d-design/software-videos.aspx , http://www.cadent.biz/  GC

Use of a Water Flosser for Interdental Cleaning

Interdental cleaning is an important part of a patient’s personal oral care regimen.  Water flossers, also known as oral irrigators (WaterPik), can play a vital, effective role in interdental hygiene. Evidence has shown a significant reduction in plaque biofilm from tooth surfaces and the reduction of subgingival pathogenic bacteria from pockets as deep as 6 millimeters with the use of water flossing.  In addition, water flossers have shown to reduce gingivitis, bleeding, probing pocket depth, and tartar buildup  related to dental implants, porcelain veneers, crowns, orthodontics, as well as natural teeth.  GC

New Patient Experience

The New Patient Exam (as well as all hygiene visits) should include an Oral Cancer Exam.  It is painless and quick – it takes only a few minutes.   The following questions should be answered for an effective oral head and neck cancer screening to take place. 

Did your dentist —

  1. Ask you to removal of any dentures (plates) or partials prior to examining your mouth?

  2. Inspect your face, neck, lips, and mouth for any signs of cancer?

  3. With both hands, feel the area under your jaw and the side of your neck, checking for lumps, bumps, or bruises that may suggest cancer?

  4. Look aand feel the insides of your lips and cheeks to check for possible signs of cancer, such as red and/or white patches?

  5. Have you stick out your tongue so it can be checked for swelling or abnormal color texture?

  6. Use gauze to gently pull your tongue to one side, and then the other, to check the base and underside of your tongue?

  7. Look at the roof and floor of your mouth, as well as the back of your throat?

  8. Put one finger on the floor of your mouth and, with the other hand under your chin, gently press down to check for lumps or sensitivity?

Porcelain Laminate Veneers

Porcelain laminate veneers enhance a person’s smile.  My patient has a past dental history of tetracycline staining and was previously restored with porcelain veneers twenty years ago.  I am currently treating him with new veneers using today’s dental materials.  He now feels more confident and is very happy with the result.  The series of YouTube videos will take you through a step by step process of converting a severely discolored and uneven smile to a more natural and attractive smile.  Proper communication is important when transferring information to the laboratory.  http://www.youtube.com/user/gmcuomo?feature=mhsn  GC

The FAU Pre-Dental Mentorship Program
